Meridian Awarded $250,000 Care Coordination Grant

Lutheran Services Health Systems (LSF) has awarded Meridian Behavioral Healthcare a $250,000 coordination grant to be used over the next year.

Meridian Behavioral Healthcare, Inc. will implement evidence-based Care Coordination services to high-need adult consumers with substance use disorder or co-occurring substance use/mental health disorders, who are also high utilizers of acute, crisis services as their primary source of behavioral health care.

The primary goals of  Meridian’s  Integrated Coordinated Care (MICC) Team is  to reduce utilization of high end services and their associated costs, while helping to stabilize participants’ lives and ultimately improve longevity and overall quality of life.

Research has shown that a large subpopulation of the same target population of consumers also experience high rates of comorbid physical disorders, including chronic conditions such as cardiovascular disease, diabetes, and respiratory disease (Walker, McGee, & Druss, 2015).

To address these needs, the MICC Team will also provide care coordination for all program participants using the primary health clinic that is currently under construction on Meridian’s campus, including the provision and coordination of medications and treatment, ensuring labs, medications and other ancillary services are provided, and coordinating referrals to specialty care.

The MICC Team will also assist consumers with coordinating and maintaining their healthcare at another primary care provider in their community if preferred. This funding will result in improving the overall wellbeing of Meridian clients and save money for the community at large.
